What to Expect During the Visit
At CHI St. Luke's Vintage, we most frequently handle medical power of attorney and advance directive notarizations for patients preparing for surgery or managing serious health conditions. The hospital's location in the Champions corridor means many of our clients here are longtime NW Houston residents whose families need documents signed while a loved one is hospitalized.
We verify each signer's identity using a valid government-issued photo ID, confirm they are signing voluntarily, and apply the notary seal. The patient must be alert, willing, and able to understand the document being signed. If there is any concern about the patient's capacity, consult with the care team before we arrive β the notary cannot assess or determine competency. Hospital staff are generally willing to help coordinate the timing of bedside visits around the patient's care schedule.
If your documents require witnesses and none are available at the hospital, we can provide them for an additional fee ($25 per witness). Do not count on hospital staff to serve as witnesses β most facilities do not allow it. Check with the attorney or party who prepared the documents to confirm whether witnesses are needed.
